Background:

  • Orbital lymphomas, often marginal-zone B-cell lymphomas, account for approximately 1% of non-Hodgkin lymphomas.
  • Diffuse large B-cell lymphoma (DLBCL) is the second most common type of orbital lymphoma and can lead to rapid vision loss if untreated.

Observation:

  • A patient presented with sudden, severe vision loss.
  • Examination revealed rubeosis iridis, hyphema, neovascular glaucoma, and vitreous hemorrhage.
  • A medical history of systemic DLBCL was noted, and an orbital mass in the optic nerve was identified.

Findings:

  • Optic nerve biopsy confirmed diffuse large B-cell lymphoma.
  • This presentation of neovascular glaucoma secondary to an extraocular DLBCL is, to the authors' knowledge, unprecedented.
