    Summary

    Giant cell arteritis and polymyalgia rheumatica patients show abnormal glucose regulation, likely due to insulin resistance. Corticosteroids may improve glucose tolerance by reducing inflammation.

    Background:

    • Giant cell arteritis (GCA), also known as Horton's disease, is a systemic vasculitis.
    • Polymyalgia rheumatica (PMR) is often associated with GCA.
    • Previous studies suggest glycoregulation abnormalities in GCA.

    Purpose of the Study:

    • To investigate glycoregulation in patients with GCA.
    • To assess pancreatic function in GCA patients.
    • To compare glycoregulation in GCA with other inflammatory conditions.

    Main Methods:

    • Oral glucose tolerance tests were performed on 8 GCA patients (6 with Horton's disease, 2 with PMR).
    • Insulinaemia and C-peptide response to glucagon were measured.
    • Results were compared to an age and weight-matched control group with other inflammatory syndromes.

    Main Results:

    • All GCA patients exhibited abnormal glucose tolerance tests (6 with diabetes mellitus, 2 with impaired glucose tolerance).
    • Elevated insulinaemia and insulin response areas were observed.
    • Normal pancreatic function and C-peptide response to glucagon suggest insulin resistance, not pancreatic dysfunction.

    Conclusions:

    • Glycoregulation disorders are common in GCA and likely due to insulin resistance.
    • The inflammatory syndrome, not just age, contributes to insulin resistance.
    • Corticosteroid treatment may normalize glucose tolerance by reducing inflammation and insulin resistance.
