Objectives:
This study aimed to evaluate the feasibility of intravoxel incoherent motion diffusion-weighted magnetic resonance imaging (DW-MRI) in detecting bowel inflammation in patients with Crohn disease (CD).
Methods:
Sixteen patients who underwent intravoxel incoherent motion DW-MRI for CD and colonoscopy were recruited. Seventy-nine bowel segments were selected, and their mean D, D*, f, and apparent diffusion coefficient (ADC) values were measured. The receiver operating characteristic curve was performed to distinguish inflamed from normal bowel.
Results:
The mean D, D*, f, and ADC values of inflamed bowel were significantly lower than those of normal bowel (P < 0.05). The area under the receiver operating characteristic curve for f (0.906) and ADC values (0.924) was greater than that for D (0.709) or D* values (0.686) for differentiating inflamed bowel from normal bowel (P < 0.05).
Conclusions:
Intravoxel incoherent motion DW-MRI is a feasible technique for detecting inflammation in CD patients. The ADC and f values have more potential than the D and D* values.
Insights
Intravoxel incoherent motion diffusion-weighted MRI (DW-MRI) effectively detects bowel inflammation in Crohn disease (CD) patients. Apparent diffusion coefficient (ADC) and f values show the most promise for identifying inflamed bowel segments.
Area of Science:
- Radiology
- Gastroenterology
- Medical Imaging
Background:
- Crohn disease (CD) is a chronic inflammatory bowel disease.
- Accurate detection of bowel inflammation is crucial for CD management.
- Current diagnostic methods may have limitations.
Purpose of the Study:
- To assess the feasibility of intravoxel incoherent motion diffusion-weighted magnetic resonance imaging (DW-MRI) for detecting bowel inflammation in CD patients.
- To compare the diagnostic performance of different DW-MRI parameters.
Main Methods:
- Sixteen CD patients underwent intravoxel incoherent motion DW-MRI and colonoscopy.
- Seventy-nine bowel segments were analyzed for mean D, D*, f, and apparent diffusion coefficient (ADC) values.
- Receiver operating characteristic (ROC) curve analysis was used to differentiate inflamed from normal bowel.
Main Results:
- Inflamed bowel segments showed significantly lower mean D, D*, f, and ADC values compared to normal segments (P < 0.05).
- The area under the ROC curve was highest for f (0.906) and ADC (0.924), indicating superior performance in differentiating inflamed from normal bowel.
- D and D* values demonstrated lower diagnostic accuracy (0.709 and 0.686, respectively).
Conclusions:
- Intravoxel incoherent motion DW-MRI is a feasible and promising technique for detecting bowel inflammation in CD.
- ADC and f values are more effective than D and D* values for distinguishing inflamed from normal bowel in CD patients.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...

A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...

Radionuclide Testing
Radionuclide testing is a sophisticated medical technique for assessing gastrointestinal motility. It focuses on gastric emptying and colonic transit time. Radioactive markers track the movement of food through the digestive system, providing insights into gastrointestinal disorders.

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
