Summary

Delays and misdiagnoses are common in neuro-ophthalmology referrals, but neuro-ophthalmologists often prevent serious vision and life-threatening complications. This study analyzed referral patterns to improve patient outcomes.

Purpose of the Study:

  • Characterize rates of misdiagnoses and delayed diagnoses.
  • Delineate outcomes following neuro-ophthalmologic evaluation.

  • Retrospective chart review of 300 new patients at a tertiary care neuro-ophthalmology clinic.
  • Collected data on demographics, travel, referral times, prior consultations, and diagnoses.
  • Analyzed referral diagnoses versus final diagnoses and impact on patient outcomes.

  • Most referrals (81%) were appropriate, but 49% of patients were at least partially misdiagnosed before referral.
  • Median time from symptom onset to neuro-ophthalmology consultation was 210 days.
  • Neuro-ophthalmologists were crucial in directing treatment, preventing complications in 21% of patients.

Conclusions:

  • Referrals to neuro-ophthalmology are often delayed, and misdiagnoses are frequent.
  • Despite delays, neuro-ophthalmologists play a vital role in preventing severe vision loss and life-threatening complications.
  • Improving referral processes can enhance patient care and outcomes.
