    Background:

    • Horizontal semicircular canal (HSC) light cupulopathy is a rare peripheral vestibular disorder.
    • Accurate diagnosis relies on identifying characteristic nystagmus patterns during specific clinical tests.

    Observation:

    • Two patients with HSC light cupulopathy presented with distinct clinical histories and manifestations.
    • Both patients exhibited typical light cupulopathy nystagmus during the Roll-Test.
    • Nystagmus in these cases was sustained towards the ground, lacking latency and attenuation at the null plane.

    Findings:

    • Direction-changing positional nystagmus and disappearance of nystagmus with head deflection (null plane) are indicative of HSC light cupulopathy.
    • The Roll-Test revealed sustained, ground-directed nystagmus without latency or attenuation in affected individuals.
