    Summary

    False positive Hepatitis B surface antigen (HBsAg) results are rising. A cutoff of 6.0 for HBsAg confirmatory testing can reduce laboratory workload and improve diagnostic accuracy.

    Background:

    • Increasing false positive Hepatitis B surface antigen (HBsAg) results due to sensitive immunoassays.
    • Need for improved laboratory efficiency and positive predictive value in HBsAg testing.

    Purpose of the Study:

    • To propose an optimized process for HBsAg testing.
    • To reduce laboratory burden and enhance the positive predictive value of HBsAg assays.

    Main Methods:

    • Analysis of samples with Elecsys HBsAg II assay (Roche Diagnostics) between cutoff index (COI) 0.9 and 10.0.
    • Application of Elecsys HBsAg Confirmatory Test for neutralization.
    • Determination of HBsAg positivity based on COI reduction after neutralization.

    Main Results:

    • 133 samples analyzed; 70.7% confirmed positive.
    • Highest COI in negatively confirmed samples was 5.6.
    • Receiver operating characteristic curve analysis showed an area under the curve of 0.761; 100% specificity at COI 6.0.

    Conclusions:

    • Propose a confirmatory testing cutoff of HBsAg COI less than 6.0.
    • This threshold aims to minimize unnecessary confirmatory tests.
    • Potential to streamline HBsAg diagnostic workflows.
