Background:

VACTERL association is a complex congenital disorder characterized by vertebral anomalies, anal atresia, cardiac defects, tracheoesophageal fistula/esophageal atresia, renal anomalies, and limb defects.
Airway abnormalities are infrequently reported in VACTERL association, making their recognition vital.

Observation:

A 10-month-old infant with a history of anal atresia presented with recurrent cough and fever.
The patient was diagnosed with VACTERL association.

Findings:

Diagnostic imaging, including bronchoscopy and chest CT with computed tomography angiography, revealed multiple airway abnormalities.
Specific findings included a bridging bronchus, airway malacia, and complete tracheal rings.

Implications:

This case highlights that multiple airway anomalies can be associated with VACTERL association.
Comprehensive evaluation of the tracheobronchial system using advanced imaging and bronchoscopy is recommended for patients with VACTERL association, particularly those with respiratory symptoms.
