Background:

The patellar tendon, crucial for knee extension, typically presents as a single structure.
Anatomical variations of the patellar tendon are documented, but extremely rare.

Observation:

A 46-year-old male presented with a 3-month history of left knee pain.
Incidental imaging findings revealed a double patellar tendon without a crossover morphology.

Findings:

One deep bundle followed a conventional patellar tendon course.
A second, distinct superficial bundle coursed anterolaterally, connecting the patella to the tibial tuberosity.

Implications:

Accurate identification of patellar tendon variations is vital for radiologists and clinicians.
This case expands the known spectrum of patellar tendon anatomical anomalies.
Understanding such rare variations aids in accurate diagnosis and safe clinical practice.
