Summary

Using Rh(D)-positive red blood cells (RBCs) in Rh(D)-negative trauma patients risks alloimmunization. This study found 17% of such patients developed new Rh antibodies, highlighting risks of RBC transfusion policies.

Background:

  • Group O Rh(D)-negative red blood cells (RBCs) are preferred for trauma patients but often scarce.
  • Transfusion guidelines permit using Rh(D)-positive RBCs to conserve Rh(D)-negative units, especially during massive transfusion protocols (MTPs).
  • The risk of alloimmunization from Rh(D)-positive RBC exposure in Rh(D)-negative individuals is not well-defined.

Purpose of the Study:

  • To evaluate the incidence of alloimmunization in Rh(D)-negative trauma patients receiving Rh(D)-positive RBCs.
  • To assess the clinical implications of current transfusion policies regarding Rh(D) compatibility in trauma.

Main Methods:

  • Retrospective review of 1,198 trauma patients with MTP activation.
  • Focused analysis on 72 Rh(D)-negative patients who received at least one unit of Rh(D)-positive RBCs.
  • Review of patient records for evidence of new Rh group antibody formation.

Main Results:

  • 17% (12 out of 72) of Rh(D)-negative patients developed new Rh group antibodies after exposure to Rh(D)-positive RBCs.
  • All alloimmunized patients formed anti-D; three also developed anti-E or anti-C.
  • Detection of novel alloantibodies was more likely with increased antibody screening frequency.

Conclusions:

  • Transfusing Rh(D)-positive RBCs to Rh(D)-negative patients carries a significant risk of alloimmunization.
  • Current transfusion policies require careful consideration due to potential patient harm.
  • RBC shortages present ongoing challenges for blood banks managing Rh(D) compatibility.
