Peripheral Arterial Disease II: Clinical Manifestations and Diagnostic Evaluation

302
Clinical manifestationsPeripheral Arterial Disease (PAD) manifests through a range of symptoms, from the characteristic intermittent claudication to atypical presentations and severe complications in advanced stages. Intermittent claudication, a hallmark symptom of PAD, presents as exercise-induced muscle pain that typically resolves within minutes of rest. This pain is reproducible and stems from inadequate blood flow, leading to the accumulation of lactic acid produced during anaerobic...

Area of Science:

  • Neurology
  • Telemedicine
  • Healthcare Management

Background:

  • Polyneuropathy (PN) is a prevalent condition causing significant morbidity.
  • Tele-polyneuropathy (tele-PN) clinics were established to enhance neurology access and guideline-adherent PN care.
  • A mixed-methods evaluation assessed pilot tele-PN clinics at three community sites.

Purpose of the Study:

  • To evaluate the feasibility and acceptability of pilot tele-PN clinics.
  • To assess the impact of tele-PN on guideline-concordant care indicators.
  • To measure patient satisfaction and gather feedback on the tele-PN experience.

Main Methods:

  • The study involved 25 patients and 48 scheduled visits.
  • Data collected included tele-PN visit/exam duration, adherence to care indicators (PN labs, opiate reduction, PT for falls), and patient satisfaction.
  • Qualitative feedback from patients and the clinical team was elicited and combined with descriptive statistics.

Main Results:

  • Average tele-PN encounter and exam times were 28.5 and 9.1 minutes.
  • Guideline-concordant care improved: PN screening labs (80% to 100%), opiate freedom (68% to 88%), PT for falls (58% to 100%).
  • 86% of patients preferred tele-PN follow-up; convenience saved an average of 231 minutes per patient in travel time.

Conclusions:

  • Pilot tele-PN clinics efficiently facilitated remote patient encounters and examinations.
  • Tele-PN successfully promoted guideline-concordant polyneuropathy care.
  • The tele-PN model provided a high-satisfaction patient experience, highlighting convenience and effective care delivery.
