Background:

Malignant glaucoma is a rare, severe form of angle-closure glaucoma.

Observation:

An 80-year-old female presented with acute vision loss and severe ocular pain in her left eye.
Initial examination revealed a shallow anterior chamber, high intraocular pressure (80 mmHg), and pseudophakia.
Despite laser iridotomy and medical management, intraocular pressure remained elevated (55 mmHg) with corneal edema.

Findings:

The patient was diagnosed with pseudophakic malignant glaucoma.
Laser YAG capsulotomy did not resolve the condition.
