Ovarian inguinal hernias are rare but should be considered in women with groin masses. Fertility preservation is crucial during surgical repair in women of reproductive age.

Purpose of the Study:

  • To review existing literature on inguinal hernias containing ovaries.
  • To summarize evidence regarding ovarian inguinal hernias with or without other female adnexa.

Main Methods:

  • Systematic literature search up to February 2018 (MEDLINE, Scopus, Google Scholar).
  • Included observational studies and case reports of ovarian inguinal hernias.
  • Focused on pre-operative and intra-operative diagnoses in women.

Main Results:

  • Evaluated 15 papers (13 case reports, 2 case series) with 17 patients (mean age 47.9 years).
  • Left-sided hernias were more common (77%).
  • Hernia repair involved repositioning (11 patients), salpingo-oophorectomy (2), or oophorectomy (2); mesh repair was common (8 patients).

Conclusions:

  • Ovarian inguinal hernias warrant consideration in the differential diagnosis of groin masses.
  • Hernia repair in women of reproductive age should prioritize fertility preservation.
