Summary

Transient ST-segment elevation after cardioversion is documented. We report new cases of intermittent ST-segment elevation, potentially due to electroshock timing differences in cardiac action potentials.

Area of Science:

  • Cardiology
  • Electrophysiology

Background:

  • Transient ST-segment elevation post-cardioversion is a known phenomenon.
  • Existing explanations include myocardial injury and coronary vasospasm.

Observation:

  • This report details two novel cases of intermittent ST-segment elevation following direct current cardioversion.
  • This specific pattern of intermittent elevation has not been previously documented.

Findings:

  • The observed intermittent ST-segment elevation is hypothesized to result from electroshock-induced timing differences in action potentials between the epicardium and endocardium.
  • This finding offers a new perspective on post-cardioversion electrocardiographic changes.

Implications:

  • This hypothesis may refine our understanding of electrocardiographic changes after cardioversion.
