Background:

Microphthalmos, a rare congenital condition, presents with a small eye.
Pure microphthalmos, also known as nanophthalmos, is characterized by reduced eye size, a high lens-to-eye volume ratio, and thickened sclera without other anomalies.

Observation:

Nanophthalmos can be sporadic or hereditary, with potential for recessive or dominant inheritance patterns.
Eyes with nanophthalmos are predisposed to angle-closure glaucoma and uveal effusion.
Uveal effusion in nanophthalmos may result from thickened sclera obstructing vortex veins or occur spontaneously.

Findings:

Familial cases of nanophthalmos associated with angle-closure glaucoma (without uveal effusion or microcornea) suggest autosomal recessive inheritance.
Angle-closure glaucoma in nanophthalmos is linked to age-related lens growth and potential choroidal detachment.
Surgical globe decompression may worsen uveal effusion.

Implications:

Understanding the genetic basis of nanophthalmos is crucial for genetic counseling and family planning.
Early detection and management of glaucoma in nanophthalmos patients are essential.
Further research into the mechanisms of uveal effusion in nanophthalmos may reveal new therapeutic targets.
