Summary

Researchers identified three microRNAs overexpressed in primary aldosteronism subtypes, bilateral adrenal hyperplasia (BAH) and aldosterone-producing adenoma (APA). While these circulating microRNAs show differences, they are not yet accurate enough for clinical diagnosis.

  • Distinguishing between bilateral adrenal hyperplasia (BAH) and aldosterone-producing adenoma (APA) is crucial for patient management.
  • Minimally invasive biomarkers are needed for accurate diagnosis and subtyping.

Purpose of the Study:

  • To investigate circulating microRNA profiles in primary aldosteronism.
  • To compare microRNA expression between BAH and APA.
  • To assess the diagnostic potential of microRNAs for subtyping primary aldosteronism.

Main Methods:

  • Plasma samples from 123 primary aldosteronism patients were analyzed.
  • Next-generation sequencing was used on 30 samples for initial discovery.
  • Real-time reverse transcription-qPCR validated findings in 93 independent samples.

Main Results:

  • Overexpression of hsa-miR-30e-5p, hsa-miR-30d-5p, and hsa-miR-7-5p was confirmed in hyperplasia compared to adenoma.
  • Aldosterone-producing adenoma samples exhibited greater microRNA expression heterogeneity than hyperplasia samples.
  • Identified microRNAs did not achieve sufficient sensitivity and specificity for clinical use.

Conclusions:

  • Specific microRNAs are differentially expressed between BAH and APA.
  • Current microRNA profiles lack the accuracy for routine clinical application in primary aldosteronism subtyping.
  • Further research may identify more robust microRNA biomarkers.
