Summary
Thermal ablation offers excellent local tumor control for low-risk papillary thyroid microcarcinomas (PTMCs). This systematic review found no local recurrence or distant metastasis, with minimal lymph node metastasis in patients undergoing thermal ablation for PTMCs.
Area of Science:
- Oncology
- Minimally Invasive Surgery
Background:
- Papillary thyroid microcarcinomas (PTMCs) are small thyroid cancers.
- Clinical studies suggest thermal ablation is effective for PTMCs.
- Systematic evaluation of thermal ablation for low-risk PTMCs is lacking.
Purpose of the Study:
- To systematically evaluate the efficacy of thermal ablation for low-risk papillary thyroid microcarcinomas.
- To assess local tumor control and adverse events following thermal ablation in PTMC patients.
Main Methods:
- Systematic review of Ovid-MEDLINE and EMBASE databases up to May 1, 2019.
- Included studies reporting thermal ablation efficacy in low-risk PTMCs.
- Data extraction and quality assessment by two radiologists following PRISMA guidelines.
Main Results:
- 503 low-risk PTMCs in 470 patients from 9 studies were analyzed.
- No local tumor recurrence or distant metastasis observed.
- 0.4% experienced lymph node metastasis; 0.2% developed new PTMC, successfully treated.
Conclusions:
- Thermal ablation is an excellent method for local tumor control in low-risk PTMCs.
- Favorable outcomes require strict inclusion criteria and technical expertise.
- Further research may refine patient selection and procedural techniques.
