Predicting proximal decay in permanent first molars is crucial. Caries on adjacent surfaces and overall caries risk are key indicators for mesial surfaces, but differ for distal surfaces.

Background:

  • Proximal surfaces of permanent first molars are susceptible to decay.
  • Understanding predictors of decay is essential for targeted prevention strategies.

Purpose of the Study:

  • To identify predictors of proximal decay in permanent first molars.
  • To analyze differences in predictors between mesial and distal surfaces.

Main Methods:

  • Cross-sectional study involving 171 patients and 227 permanent first molars.
  • Calibrated examiners collected detailed patient history and performed examinations.
  • Backward regression analysis was used to identify significant caries predictors (p ≤ 0.05).

Main Results:

  • Mesial surfaces showed higher decay rates (0.92 ± 0.85).
  • Caries risk profile (60%) and adjacent surface caries (90%) predicted mesial decay.
  • Only caries risk (3%) predicted distal decay, indicating differing predictors.

Conclusions:

  • Adjacent tooth surface caries and caries risk profile are significant predictors for mesial surfaces.
  • Predictors for mesial and distal surfaces of permanent first molars may differ, necessitating individualized risk assessment.
