Background:

Cataract surgery is the most common global procedure.
Small pupils (miosis) present a significant surgical challenge, increasing complication risks like capsule rupture and vitreous loss.

Purpose of the Study:

To review methods for pupil dilation during cataract surgery.
To identify risk factors associated with small pupils in cataract patients.

Main Methods:

Literature review of studies on pupil dilation and small pupil risk factors in cataract surgery.
Analysis of pharmacological, mechanical, and device-based dilation techniques.

Main Results:

Identified risk factors for small pupils include diabetes, intraoperative floppy iris syndrome, pseudoexfoliation, glaucoma medications, prior ocular surgery, and age-related iris changes.
Management options include pharmacological agents, mechanical stretching, iris hooks, and pupil expanders.

Conclusions:

Effective management of small pupils is crucial for cataract surgeons.
A stepwise approach, from pharmacologic agents to pupil expanders, is recommended for intraoperative pupil dilation.
