Background:

  • Existing scoring systems for renal tumor surgery have limitations in predicting surgical approach.

Purpose of the Study:

  • To select optimal elements from established scoring systems (RENAL, PADUA, DAP, ZS, NephRO, ABC, CI) to create improved predictive models for surgery type.
  • To develop and validate novel models for predicting surgical approach in renal tumor cases.

Main Methods:

  • Sixteen elements from seven systems were categorized into six groups (e.g., tumor size, collecting system involvement, invasion depth).
  • 320 new models were constructed by combining three elements from different categories.
  • Optimized models were identified based on Area Under the Curve (AUC) analysis.

Main Results:

  • Three optimized models achieved an AUC of 0.88, outperforming the original seven systems (AUC 0.81-0.844).
  • Key predictive elements included tumor characteristics like "exophytic/endophytic" properties and "correlation with collecting system or sinus."
  • The category "correlation with collecting system or sinus" was significantly associated with predicting surgery type (p = 0.001).

Conclusions:

  • Established scoring systems demonstrate good predictive power for surgery type.
  • Three novel, optimized models offer enhanced prediction accuracy for surgical approach.
