Background:

Developing effective community care requires integrated health and social needs assessments.
Standardized frameworks like the International Classification of Functioning, Disability, and Health (ICF) can improve care coordination.
Information and Communication Technology (ICT) offers potential for enhancing assessment processes.

Observation:

A novel ICT-based Comprehensive Health and Social-Needs Assessment (CHSNA) system was designed.
The system integrates the ICF framework into its assessment structure.
Expert validation via the Delphi method and a pilot test were conducted.

Findings:

The CHSNA system features a user-friendly interface with three assessment steps: basic health, life and activity, and in-depth health.
Visualized health profiles, including images, graphs, and an ICF model, are generated for community residents.
The system supports the evaluation of health and social needs and facilitates tailored healthcare plans.

Implications:

The CHSNA system can improve the accuracy of needs assessments for community residents.
It facilitates better identification of appropriate healthcare plans and services.
Standardized terminology through ICF integration enhances care consistency and professional support.
