Tuberculosis, often called TB, is a contagious illness primarily caused by Mycobacterium tuberculosis. It mainly affects the lung parenchyma but can also impact other body parts.
Causative Organism
The primary infectious agent causing tuberculosis is Mycobacterium tuberculosis, a slow-growing, acid-fast, aerobic rod that exhibits sensitivity to heat and ultraviolet light. Instances of Mycobacterium bovis and Mycobacterium avium contributing to the development of TB infection are rare.

Summary

Diagnosing intestinal tuberculosis using stool GeneXpert assay is a rapid alternative. This method shows good accuracy, aiding early detection and management of intestinal tuberculosis.

Background:

  • Diagnosing intestinal tuberculosis is difficult due to low yields from CT scans and colonoscopies.
  • GeneXpert MTB/RIF (Xpert) assays show promise for diagnosing tuberculosis in various body secretions.

Purpose of the Study:

  • To evaluate the diagnostic yield and accuracy of the GeneXpert assay when performed on stool samples from patients with suspected intestinal tuberculosis.

Main Methods:

  • One hundred patients underwent standard diagnostic tests including colonoscopy and biopsies.
  • Fresh stool samples were collected and tested using the GeneXpert assay for Mycobacterium tuberculosis and Rifampicin resistance.

Main Results:

  • GeneXpert assay on stool was positive in 20% of suspected intestinal tuberculosis cases.
  • Compared to histopathology, stool GeneXpert showed a sensitivity of 39.1% and specificity of 85.7%.

Conclusions:

  • Stool GeneXpert assay provides a rapid alternative for detecting intestinal tuberculosis with acceptable accuracy.
  • While not replacing AFB culture or histopathology, it can contribute to earlier diagnosis and treatment initiation.
