Background:

A 67-year-old female with a history of mitral prosthesis presented with symptoms of heart failure.
The patient exhibited mild to moderate pulmonary edema and severe right heart failure.
Associated conditions included pulmonary and tricuspid insufficiency.

Observation:

Clinical examination and noninvasive tests did not reveal prosthetic valve dysfunction.
Cardiac catheterization showed a significant V wave.
No mitral regurgitation or significant mitral diastolic gradient was detected.

Findings:

A diagnosis of stiff left atrium syndrome was made.
The condition was confirmed post-mortem.
The pathophysiology of stiff left atrium syndrome was elucidated.

Implications:

This case highlights a rare cause of heart failure in patients with prosthetic valves.
Understanding stiff left atrium syndrome is crucial for accurate diagnosis and management.
Further research into the pathophysiology may improve patient outcomes.
