Caloric testing nystagmus duration is shorter than recommended, suggesting shorter intervals between warm and cool irrigations can improve efficiency. This study provides evidence for optimizing vestibular assessment times.

Background:

Purpose of the Study:

  • To measure nystagmus duration following warm and cool caloric irrigations.
  • To provide evidence for optimal intervals between caloric irrigation onsets.
  • To compare nystagmus durations and maximum slow phase velocity (SPV) between warm and cool irrigations.

Main Methods:

  • Participants underwent up to four caloric irrigations.
  • Nystagmus duration and maximum SPV were measured until nystagmus returned to baseline levels (within 2°/s).
  • Data from 52 vestibular assessment patients (99 ears) were analyzed.

Main Results:

  • Mean nystagmus duration was 3:04 minutes (183.9 seconds) from irrigation onset.
  • Nystagmus returned to minimal levels within 4:25 minutes (264.8 seconds) in 97.5% of participants.
  • No significant difference in duration or SPV was found between warm and cool irrigations.

  • Nystagmus duration is substantially shorter than the currently recommended 7-minute interval between caloric irrigations.
  • Findings suggest shorter intervals are feasible, regardless of irrigation temperature or SPV.
  • Reducing caloric testing time can improve clinical efficiency in vestibular assessments.
