    • Renal transplantation is a life-saving procedure for end-stage renal disease.
    • Allograft rupture is a rare but serious complication following kidney transplantation.

    Observation:

    • Nine cases of spontaneous allograft rupture were observed in 325 renal transplantations.
    • Graft repair was successful in 4 instances, with one functioning for over five years.
    • Immediate nephrectomy was required in 5 cases due to irreversible rejection or hemorrhage.

    Findings:

    • Severe acute rejection was a consistent finding in all 9 cases of allograft rupture.
    • The duration of ischemia and preservation methods did not appear to be causative factors.
    • A correlation was noted between the rising incidence of graft rupture and increased severe early acute rejection.

    Implications:

    • Aggressive management of early acute rejection is crucial to prevent spontaneous allograft rupture.
    • Understanding the link between rejection and rupture can improve post-transplant monitoring protocols.
    • Further research into the mechanisms of rejection-induced rupture may lead to novel therapeutic strategies.
