Rheumatoid arthritis and Porphyromonas gingivalis infection impair bone healing. Combined, they synergistically worsen fracture healing and reduce bone mineral density, indicating risk for prolonged recovery.

  • Porphyromonas gingivalis infection is linked to periodontitis, dysbiosis, and rheumatoid arthritis (RA).
  • Rheumatoid arthritis and oral dysbiosis are recognized risk factors for impaired bone regeneration.

Purpose of the Study:

  • To investigate the impact of P. gingivalis administration on bone regeneration in mice with or without collagen-induced arthritis (CIA).

Main Methods:

  • Male DBA/1 mice were sensitized to CIA and/or administered P. gingivalis, followed by bilateral femur drilling.
  • Bone regeneration was assessed via micro-computed tomography (micro-CT) for bone volume (BVd/TVd), osteoclast counts, and osteoblast surface (Ob.S/BS).
  • Serum cytokine levels and tibial bone mineral density were also measured.

  • CIA significantly reduced bone regeneration at all time points. P. gingivalis alone showed similar effects at 2 weeks.
  • CIA increased osteoclast numbers and decreased osteoblast surfaces. P. gingivalis decreased osteoblast surfaces at 2 weeks.
  • Combined CIA and P. gingivalis synergistically decreased bone regeneration, increased TNF-α, and lowered bone mineral density.

Conclusions:

  • Rheumatoid arthritis and P. gingivalis-induced dysbiosis act as risk factors for delayed fracture healing.
  • The synergistic effect of RA and dysbiosis significantly impairs bone regeneration and bone mineral density.
