Background:

  • The impact of a term CS on subsequent pregnancy outcomes, particularly spontaneous preterm birth (PTB), requires further investigation.

Purpose of the Study:

  • To determine the risk of overall PTB and spontaneous PTB in a pregnancy following a caesarean section (CS) performed at term.
  • To analyze the influence of CS type (planned vs. unplanned) on subsequent PTB risk.

Main Methods:

  • A longitudinal linked national cohort study utilizing data from the Dutch Perinatal Registry (1999-2009).
  • Inclusion of 268,495 women with two subsequent singleton pregnancies.
  • Subgroup analysis based on gestational age at first delivery and type of CS.

Main Results:

  • Women with a previous term CS had a slightly higher incidence of PTB in their second pregnancy compared to those with a previous vaginal delivery (2.79% vs. 2.46%).
  • The increased risk was primarily driven by spontaneous PTB after a previous term CS (aOR 1.50).
  • Both planned (aOR 1.86) and unplanned (aOR 1.40) term CS were associated with an increased risk of spontaneous PTB in the next pregnancy.

Conclusions:

  • Term caesarean section is associated with a marginally increased risk of spontaneous preterm birth in a subsequent pregnancy.
  • The type of caesarean section (planned or unplanned) influences the magnitude of this increased risk.
