Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

462
Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Assessment of the Rectum and Anus

967
Evaluating the rectum and anus plays a crucial role in conducting a thorough physical examination of the gastrointestinal system. Although it may be uncomfortable and often embarrassing for the patient, it holds immense diagnostic value, particularly in detecting gastrointestinal diseases and abnormalities. This guide will explain how to perform this assessment using inspection and palpation methods.
Rectal Inspection
Begin by inspecting the perianal and anal areas for color, texture, rashes,...

Inflammatory Bowel Disease V: Surgical Management

403
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Lower GI Series: Barium Enema

1.1K
A Barium Enema, or a lower GI series, is a specialized radiographic examination designed to visualize the lower gastrointestinal tract, specifically the colon and rectum. This procedure is instrumental in diagnosing various conditions such as colorectal cancer, polyps, diverticulosis, and inflammatory bowel disease.
Procedure Details
The examination begins by inserting a lubricated rectal tube into the patient's rectum to administer a radiopaque barium solution. The barium flow is carefully...

Imaging Studies III: Gastrointestinal Motility Studies and Virtual Colonoscopy

321
This lesson explores three gastrointestinal imaging techniques: radionuclide testing, colonic transit studies, and virtual colonoscopy.
Radionuclide Testing
Radionuclide testing is a sophisticated medical technique for assessing gastrointestinal motility. It focuses on gastric emptying and colonic transit time. Radioactive markers track the movement of food through the digestive system, providing insights into gastrointestinal disorders.
In gastric emptying studies, a meal's liquid and...

Area of Science:

  • Gastroenterology
  • Medical Diagnostics

Background:

  • Predictors for small bowel stenosis beyond obstruction symptoms are not well-established.
  • Small bowel stenosis diagnosis often relies on invasive procedures.

Purpose of the Study:

  • To identify predictors of small bowel stenosis using balloon-assisted enteroscopy.
  • To improve diagnostic accuracy and patient selection for enteroscopy.

Main Methods:

  • Retrospective analysis of 461 patients undergoing enteroscopy for suspected small intestinal disease over 6 years.
  • Multivariate analysis to identify predictors of small bowel stenosis, defined as a stricture impassable by the over-tube.

Main Results:

  • Of 314 patients with a definite diagnosis, 59 (18.8%) had small bowel stenosis.
  • Predictive factors for stenosis included age ≥ 60 years (OR 2.1), underweight (BMI ≤ 18.5) (OR 3.4), and obstruction symptoms (OR 3.6).
  • Overt small bowel bleeding was associated with a lower likelihood of stenosis (OR 0.5).

Conclusions:

  • Small bowel stenosis is associated with specific patient demographics and clinical presentations.
  • Age, nutritional status, and presence of obstruction symptoms are key indicators.
  • Absence of overt small bowel bleeding may also suggest stenosis.
