Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

1.1K
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Muscles of the Forearm that Move the Hand and Fingers

2.3K
The muscles of the forearm that move the wrist, hand, and digits are numerous and diverse. They can be classified into two groups based on their location and function — the anterior and posterior compartment muscles.
Anterior Compartment
The anterior compartment muscles originate from the humerus. They primarily function as flexors and are also known as flexor muscles. They typically insert on the carpals, metacarpals, and phalanges. The superficial layer includes the flexor carpi...

Area of Science:

  • Anesthesiology
  • Orthopedic Surgery
  • Microsurgery

Background:

  • Peripheral nerve blocks are crucial for effective anesthesia in hand surgery.
  • Distal nerve blocks offer a targeted approach to anesthesia for various hand procedures.

Purpose of the Study:

  • To evaluate the efficacy and safety of distal peripheral nerve blocks in hand surgery.
  • To analyze the distribution of nerve blocks across different hand innervation regions.

Main Methods:

  • Retrospective analysis of 75 hand surgeries performed between 2015 and 2018.
  • Utilized distal nerve blocks for anesthesia, targeting median, radial, and ulnar nerve innervation areas.
  • Included identification of landmarks, skin disinfection, local anesthetic injection, and surgical procedure.

Main Results:

  • 60% of cases involved median nerve blocks, 28% radial, and 5% ulnar nerve blocks.
  • Anesthesia of two adjacent innervation areas was required in 7% of cases.
  • Distal nerve blocks demonstrated a reliable anesthetic effect with a low incidence of complications.

Conclusions:

  • Distal peripheral nerve blocks are a safe and effective anesthetic technique for a wide range of hand surgeries.
  • The technique provides reliable anesthesia with minimal complications, supporting its use in clinical practice.
