Summary

Systemic sclerosis (SSc) patients often experience subclinical cognitive impairment, even without neurological symptoms. This impairment correlates with disease severity, particularly the Medsger vascular score, highlighting the need for early detection.

Background:

  • Systemic sclerosis (SSc) is a complex autoimmune disease.
  • Cognitive dysfunction is increasingly recognized in SSc patients.
  • Subclinical cognitive impairment may precede overt neurological manifestations.

Purpose of the Study:

  • To investigate cognitive dysfunction in adult SSc patients without clinical neurological signs.
  • To correlate cognitive function with disease severity parameters in SSc.

Main Methods:

  • 20 female SSc patients and 20 healthy controls were assessed.
  • Cognitive function was evaluated using Mini-Mental State Examination (MMSE), Wechsler Adult Intelligence Scale (WAIS-III), and P300 event-related potentials (ERPs).
  • Disease severity was assessed using the Medsger severity vascular score.

Main Results:

  • 65% of SSc patients showed cognitive impairment (MMSE < 25).
  • SSc patients exhibited significantly lower MMSE scores, higher Deterioration Index (DI), and prolonged P300 latency compared to controls.
  • Cognitive impairment, indicated by DI, positively correlated with the Medsger severity vascular score.

Conclusions:

  • Subclinical cognitive impairment is prevalent in SSc patients.
  • Early subjective (MMSE, WAIS-III) and objective (P300 ERPs) assessments are recommended for diagnosing cognitive impairment in SSc.
  • The Medsger vascular score is a significant correlate of cognitive dysfunction in SSc.
