Background:

  • Pleural fluid glucose (PFG) is crucial for diagnosing and managing pleural effusions.
  • Limited research exists on point-of-care PFG testing, particularly bedside glucometer use.

Purpose of the Study:

  • To evaluate the accuracy of bedside PFG measurement using a glucometer (B-PFG) compared to standard laboratory PFG measurement (Lab-PFG).

Main Methods:

  • Patients undergoing thoracentesis or thoracostomy were enrolled.
  • PFG was measured bedside using a glucometer and in the laboratory.
  • Pearson correlation and Bland-Altman analysis compared B-PFG and Lab-PFG.

Main Results:

  • A strong correlation (r=0.98) was found between B-PFG and Lab-PFG.
  • Good agreement was observed (mean difference 14.8 mg/dL), especially for glucose <80 mg/dL.
  • Sixty patients with exudative effusions were included.

Conclusions:

  • Bedside glucometer measurement of PFG demonstrates high accuracy compared to laboratory methods.
  • Further research is warranted before clinical implementation of bedside PFG testing.
