Background:

  • Emergent transvenous (TV) pacemaker placement is a critical, life-saving procedure.
  • This infrequent procedure presents challenges for emergency medicine (EM) trainee education due to associated complications.

Purpose of the Study:

  • To develop and evaluate an inexpensive, conceptually-focused training model for transvenous (TV) pacemaker placement.
  • To assess the impact of the training model on emergency medicine resident confidence.

Main Methods:

  • A low-cost ($51) training model was constructed using polyvinyl chloride pipes, vinyl tubing, and a submersible pump.
  • Fifteen EM residents participated in a training session using the model.
  • Participant confidence was measured before and after the session using a survey and compared with a Wilcoxon matched-pairs test.

Main Results:

  • Median confidence in TV pacemaker placement significantly increased from 2 (minimally confident) to 4 (very confident) post-training (p=0.001).
  • All participants agreed the model enhanced their understanding of the procedure.
  • The model provided direct visualization for practicing a complex emergency procedure.

Conclusions:

  • An inexpensive, visualized training model effectively improves trainee confidence in transvenous (TV) pacemaker placement.
  • This simulation tool offers a valuable method for teaching a complex and infrequent emergency procedure.
  • Direct visualization in simulation enhances understanding and confidence for critical interventions.
