Acute Pyelonephritis II: Diagnostic Studies and Management

231
Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

Acute Pyelonephritis I: Introduction

385
Pyelonephritis is a bacterial infection that primarily affects the renal parenchyma and collecting system, including the renal pelvis, tubules, and interstitial tissue of one or both kidneys. It can be classified as either acute—a sudden, severe infection—or chronic, which refers to long-term or recurrent kidney infections. The primary cause of acute pyelonephritis (APN) is bacterial infection, with Escherichia coli accounting for approximately 70-80% of cases. Other bacteria, such...

Area of Science:

  • Immunology
  • Rheumatology
  • Pediatric Osteology

Background:

  • Chronic recurrent multifocal osteomyelitis (CRMO) is a rare autoinflammatory disorder characterized by recurrent bone inflammation.
  • Delayed diagnosis can lead to persistent symptoms, joint damage, and reduced quality of life.

Observation:

  • A 16-year-old male presented with hip and knee pain.
  • Clinical and radiological evaluation revealed bone lesions consistent with CRMO.
  • No microbial growth was observed, and antibiotic treatment was ineffective.

Findings:

  • The patient was diagnosed with CRMO.
  • Treatment with prednisolone and methotrexate resulted in significant improvement.
  • Initial treatment with ibuprofen was ineffective.

Implications:

  • CRMO should be considered in the differential diagnosis of pediatric patients with joint pain and bone lesions.
  • Prompt diagnosis and appropriate immunosuppressive therapy are crucial for managing CRMO.
  • This case highlights the importance of a multidisciplinary approach in diagnosing and treating rare autoinflammatory bone diseases.
