Primary mediastinal malignant germ cell tumours (PMMGCT) are rare and challenging to manage. A multidisciplinary approach and detailed clinicopathological assessment are crucial for effective PMMGCT treatment and prognosis.
Area of Science:
- Oncology
- Thoracic Surgery
- Pathology
Background:
- Primary mediastinal malignant germ cell tumours (PMMGCT) are exceptionally rare neoplasms.
- Current management strategies for PMMGCT are often hindered by a lack of established guidelines and a generally poor prognosis.
Purpose of the Study:
- To retrospectively analyze the clinical and pathological characteristics of PMMGCT cases.
- To evaluate the effectiveness of multimodality treatment approaches and patient follow-up for PMMGCT.
Main Methods:
- Retrospective evaluation of all PMMGCT cases diagnosed and treated between 2014 and 2018.
- Analysis included clinico-pathological features, treatment modalities, and follow-up data.
Main Results:
- Five PMMGCT cases were identified: three seminomas and two non-seminomas (one yolk-sac, one mixed).
- Most cases were locally advanced; one presented with supraclavicular lymph node metastasis.
- Treatment involved platinum-based chemotherapy, with surgery for non-seminomas and radiotherapy for an inoperable seminoma case.
Conclusions:
- PMMGCT necessitates a multidisciplinary approach for optimal diagnosis and management.
- Tumor site, extension, histopathology, stage, and serum markers are vital for prognostication and treatment planning.
