Background:

Malignant systemic mastocytosis (MSM) is a rare neoplastic proliferation of mast cells.
Diagnosis can be challenging, particularly in adults without skin lesions.

A patient without cutaneous involvement presented with unsuspected MSM.
Hypotension during laparotomy indicated severe vasoactive mediator release.
