    Background:

    • Acquired reactive perforating collagenosis (ARPC) is a rare dermatologic disorder characterized by transepidermal elimination of dermal material.
    • ARPC is frequently associated with systemic conditions, notably diabetes mellitus and chronic renal failure.

    Observation:

    • A 70-year-old female with a 20-year history of poorly controlled type 2 diabetes presented with pruritic papules and nodules on her arm.
    • Histologic examination confirmed acquired reactive perforating collagenosis.

    Findings:

    • Optimal management requires collaboration between endocrinologists and dermatologists.
    • Controlling the underlying systemic disease (diabetes) and utilizing topical therapies (betamethasone, ammonium lactate, pimecrolimus) led to symptom resolution.
    • The patient's hemoglobin A1c improved following medication adjustments by her endocrinologist.

    Implications:

    • Endocrinologists play a crucial role in identifying ARPC through skin examinations.
    • Early detection and prompt referral to dermatology are vital for managing ARPC and improving patient quality of life.
