Background:

  • Malignant otitis externa (MOE) is a severe infection, often caused by Pseudomonas aeruginosa, posing significant morbidity and mortality risks.
  • Advanced and relapsing cases of MOE present treatment challenges, necessitating comprehensive management strategies.

Observation:

  • Two patients with advanced, relapsing malignant otitis externa were treated.
  • Following initial parenteral antibiotic courses, oral ciprofloxacin was administered for six months.

Findings:

  • Both patients achieved complete remission of the disease after the extended oral ciprofloxacin therapy.
  • The regimen included strict diabetic control, aural toilet, gentamicin ear drops, parenteral antibiotics (gentamicin, azlocillin), metronidazole, and a six-month course of oral ciprofloxacin (750 mg twice daily).
