Background:

  • Malignant otitis externa (MOE) typically affects elderly, diabetic patients.
  • Pseudomonas aeruginosa is the most common causative pathogen in MOE.

Observation:

  • A rare case of MOE in a fit, 25-year-old, non-diabetic male is presented.
  • The condition was bilateral, affecting both ears.
  • Clinical and histological findings supported the diagnosis of MOE.

Findings:

  • Pseudomonas aeruginosa was not identified or cultured.
  • Staphylococcus aureus was the only pathogen isolated.
  • Staphylococcus aureus has been implicated in only two prior MOE cases.

Implications:

  • This case expands the known demographic profile of MOE.
  • It highlights Staphylococcus aureus as a potential, albeit rare, pathogen in MOE.
  • Further research may be needed to understand the pathogenesis of MOE caused by Staphylococcus aureus.
