Summary

Factor D metabolism significantly slows in end-stage renal failure, with its fractional metabolic rate decreasing dramatically. While plasma levels correlate with kidney function, they are not sensitive indicators due to variable synthesis rates.

Area of Science:

  • Biochemistry
  • Renal Physiology
  • Immunology

Background:

  • Factor D is a key enzyme in the alternative complement pathway.
  • Plasma Factor D levels increase significantly in end-stage renal failure (ESRF).

Purpose of the Study:

  • To investigate the metabolism and distribution of Factor D in humans with varying renal function.
  • To determine if Factor D levels can serve as a sensitive marker for renal function.

Main Methods:

  • Administered radiolabeled Factor D to healthy individuals and patients with renal disease.
  • Utilized compartmental modeling to calculate fractional metabolic rates (FMR) and extravascular/intravascular distributions (EV/IV).
  • Correlated Factor D parameters with creatinine clearance and other clinical markers.

Related Experiment Videos

Main Results:

  • Factor D fractional metabolic rate was markedly reduced in ESRF patients compared to healthy individuals.
  • Factor D FMR correlated strongly with creatinine clearance.
  • Factor D plasma levels showed an inverse correlation with creatinine clearance but were not sensitive indicators of renal function due to wide variations in synthesis rate.
  • Urinary excretion of intact Factor D increased in patients with tubular dysfunction.

Conclusions:

  • Factor D metabolism is significantly impaired in renal failure.
  • Factor D plasma levels are not reliable indicators of renal function.
  • Factor D is filtered by glomeruli and catabolized by tubular cells in healthy individuals.
