Exhaled nitric oxide (FeNO) can predict inhaled corticosteroid (ICS) response in COPD patients. High FeNO levels indicate a good response to steroid therapy, improving symptoms and lung function.
Area of Science:
- Pulmonology
- Biomarkers
- Inflammatory Diseases
Background:
- Chronic obstructive pulmonary disease (COPD) is a complex inflammatory lung condition.
- Identifying patients who benefit from anti-inflammatory treatments like inhaled corticosteroids (ICS) is crucial.
- Predictive biomarkers for ICS response in COPD are needed.
Purpose of the Study:
- To assess inflammatory biomarkers for predicting ICS treatment response in COPD.
- To correlate biomarker levels with changes in COPD symptoms and lung function.
Main Methods:
- Prospective study of 43 symptomatic COPD patients receiving ciclesonide (ICS) plus bronchodilators.
- Measured blood eosinophils, exhaled nitric oxide (FeNO), alveolar nitric oxide (Calv), IgE, and atopy.
- Assessed changes in COPD Assessment Test (CAT) scores and FEV1 at 12 weeks.
Main Results:
- FeNO demonstrated high predictive value (AUC 0.92) for improvements in CAT score and FEV1.
- Specific FeNO cutoffs identified high likelihood (35 ppb) and low likelihood (20 ppb) of response.
- Baseline FeNO strongly correlated with improvements in FEV1 and CAT scores (p < 0.0001).
Conclusions:
- Exhaled nitric oxide (FeNO) is a valuable biomarker for predicting ICS response in COPD.
- FeNO can identify patients likely to improve symptomatically and in lung function with steroid therapy.
- This biomarker aids in personalized treatment strategies for symptomatic COPD.
