Purpose of the Study:

Main Methods:

  • Systematic search of PubMed, EMBASE, Cochrane Library, and Web of Science databases (Jan 2001–Aug 2017).
  • Inclusion of randomized clinical trials evaluating BLVR for emphysema.
  • Application of random-effects network meta-analysis within a Bayesian framework, with GRADE assessment for evidence quality.

Main Results:

  • 13 trials with 1993 participants were included; evidence quality was mostly moderate.
  • Medical care (MC) had the lowest adverse events compared to intrabronchial valve (IBV), endobronchial valve (EBV), lung volume reduction coils (LVRC), emphysematous lung sealant (ELS), and airway bypass.
  • Bronchoscopic thermal vapor ablation (BTVA) improved FEV1 versus MC, IBV, and LVRC. EBV showed improvements in FEV1, 6-minute walking distance (6MWD), and St. George's Respiratory Questionnaire (SGRQ) compared to MC, with lower mortality.

Conclusions:

  • BLVR provides significant advantages for emphysema patients.
  • Endobronchial valve (EBV) therapy demonstrates notable benefits in improving lung function, exercise capacity, and quality of life.
  • EBV was associated with reduced mortality compared to medical care, highlighting its potential as a preferred BLVR strategy.
