Summary

Severe brain hemorrhages occurred in six alcoholic patients with liver damage and bleeding disorders. These cases highlight a critical, often fatal, complication of alcoholism not typically associated with these specific bleeding patterns.

Background:

  • Alcoholism is associated with various neurological complications.

Observation:

  • Six patients with alcoholism presented with extensive cerebral hemispheric hemorrhages.
  • All patients exhibited liver damage, thrombocytopenia, and abnormal coagulation profiles (prothrombin time, partial thromboplastin time).
  • Clinical presentations included seizures, coma with and without focal deficits, and delayed deterioration.

Findings:

  • Computed tomography revealed diffuse cerebral hemorrhages with intraventricular and subarachnoid blood in all patients.
  • Autopsies in three patients excluded common causes like aneurysms, vascular malformations, or tumors.
  • Marked mass effect with minimal edema was noted, and all patients died.

Implications:

  • Alcohol-related liver disease may predispose to severe intracranial hemorrhage through coagulopathy.
  • These hemorrhages represent a distinct and often fatal neurological complication of alcoholism.
  • Further research is needed to understand the specific mechanisms and identify potential interventions.
