The number of elderly people living with HIV is rising, with a significant portion of new diagnoses in the US occurring in those over 50. This study reveals a 15.79% pooled HIV prevalence in the elderly, with low viral suppression rates.
Area of Science:
- Gerontology
- Infectious Diseases
- Public Health
Background:
- The global population of elderly individuals living with HIV (Human Immunodeficiency Virus) is increasing.
- Delayed diagnosis and treatment in this demographic are common due to co-occurring life-threatening diseases and healthcare system factors.
- Nearly half of new HIV diagnoses in the United States are among individuals over 50 years old.
Purpose of the Study:
- To determine the pooled prevalence of HIV in the elderly population.
- To assess the current continuum of care for elderly HIV patients.
- To identify challenges in HIV management for older adults.
Main Methods:
- Systematic review and meta-analysis of 28 studies published between 2000 and 2018.
- Data retrieval from major scientific databases including MEDLINE, PubMed, Cochrane Library, EMBASE, and Google Scholar.
- Statistical analysis using DerSimonian and Laird Random Effects model, STATA 13.0, and Q-GIS for mapping, with heterogeneity and bias assessed by I² statistics.
Main Results:
- The pooled prevalence of HIV in the elderly population was found to be 15.79%.
- Antiretroviral therapy (ART) coverage was moderate at 38.64%, with a low viral suppression rate of 11.52%.
- ART retention rates were 12.77%, and mortality was 6.15%, with challenges including drug interactions and comorbidities.
Conclusions:
- Elderly HIV patients face significant challenges in achieving viral suppression, despite ART availability.
- Comorbidities and drug interactions negatively impact treatment outcomes and lifespan in this population.
- Improved healthcare strategies are needed to enhance HIV care continuum for aging individuals.
