Purpose of the Study:

  • To investigate the short-term effects of aerobic exercise on liver metabolism in obese adults with NAFLD.
  • To determine the immediacy of exercise intervention on hepatic insulin extraction (HIE) and insulin sensitivity.

Main Methods:

  • A 7-day treadmill walking intervention (60 min/day) at 80%-85% maximal heart rate was conducted.
  • Participants were obese adults with >5% hepatic lipid content.
  • Insulin sensitivity and HIE were assessed using oral glucose tolerance tests and calculations based on insulin and C-peptide levels.

Main Results:

  • Aerobic exercise significantly increased HIE by 9.8% and insulin sensitivity by 34% (P < 0.05).
  • Basal fat oxidation increased, while carbohydrate oxidation decreased post-intervention (P < 0.05).
  • HIE positively correlated with adiponectin and negatively with TNF-α.

Conclusions:

  • Aerobic exercise training rapidly reverses key physiological mechanisms in NAFLD, including improved HIE and peripheral insulin sensitivity.
  • These improvements occur independently of weight loss, suggesting direct metabolic benefits.
  • Adipokine-related pathways may mediate these exercise-induced reversals in NAFLD.
