Background:

  • Glaucoma is a complex optic neuropathy causing retinal ganglion cell loss and optic nerve atrophy.
  • Acupuncture is explored as a complementary therapy, based on TCM principles of energy flow (Chi).

Purpose of the Study:

  • To evaluate the effectiveness and safety of acupuncture for glaucoma patients compared to other treatments, placebo, or no treatment.

Main Methods:

  • Comprehensive search of multiple databases for randomized controlled trials (RCTs) involving acupuncture for glaucoma.
  • Inclusion criteria focused on RCTs with at least one arm using acupuncture.
  • Data extraction and risk of bias assessment were performed independently by two reviewers.

Main Results:

  • Three completed trials and one ongoing trial were included, with varying interventions and high risk of bias.
  • Evidence certainty was very low due to bias, heterogeneity, and imprecision.
  • Some trials showed potential reductions in intraocular pressure (IOP) and visual acuity changes, but with significant limitations and missing data.

Conclusions:

  • Reliable conclusions on acupuncture's efficacy for glaucoma cannot be made due to insufficient data.
  • Ethical considerations may preclude future RCTs comparing acupuncture against standard care or placebo in established healthcare settings.
