Background:

  • Obstructive sleep apnea (OSA) is a common condition, and its positional component (POSA) may affect treatment strategies.
  • Elderly individuals represent a growing population with unique health considerations, including sleep disorders.

Purpose of the Study:

  • To determine the prevalence of position-dependent obstructive sleep apnea (POSA) in elderly patients (≥65 years).
  • To compare POSA incidence between adult (19-65 years) and elderly patient groups.

Main Methods:

  • A prospective, bi-center study involving 434 participants from January 2018 to May 2019.
  • Home polysomnography (PSG) using Embletta MPR to record sleep and body position.
  • Patients classified into adult and elderly groups; POSA defined using Cartwright, Bignold, and Amsterdam Positional OSA Classification (APOC) systems.

Main Results:

  • POSA prevalence in elderly patients varied by classification: 49.3% (Cartwright), 20.5% (Bignold).

Conclusions:

  • A substantial proportion of elderly patients may benefit from positional therapy for POSA, particularly when assessed with the APOC system.
  • Further research is essential to validate the efficacy and applicability of positional therapy in the elderly population.
