Dentin Staining Caused by Nano-silver Fluoride: A Comparative Study.

    Summary

    New nano-silver fluoride (NSF) formulations show significantly less dentin staining compared to existing commercial agents. This finding suggests NSF as a promising alternative for caries management without compromising tooth aesthetics.

    Background:

    • Silver diamine fluoride (SDF) is effective for caries arrest but can cause significant tooth staining.
    • There is a need for cariostatic agents with improved esthetic outcomes.
    • Nano-silver fluoride (NSF) formulations have been developed to potentially mitigate staining associated with silver-based treatments.

    Purpose of the Study:

    • To evaluate and compare the dentin staining potential of two new nano-silver fluoride (NSF) formulations (600 and 1500 ppm) against three commercially available cariostatic agents.
    • To assess the esthetic impact of NSF in comparison to established silver fluoride products.

    Main Methods:

    • Seventy-five extracted human molars were prepared to expose dentin.
    • Samples were treated with either NSF (600 or 1500 ppm), Advantage Arrest, Riva Star, or Cariestop (n=15 per group).
    • Dentin color change (ΔE) and lightness (ΔL) were measured using a spectrophotometer at baseline, 2 weeks, and 4 weeks; photographic analysis was also conducted.

    Main Results:

    • NSF 600 ppm and 1500 ppm exhibited the lowest color change (ΔE=1.02 and 1.53, respectively) and minimal dentin staining (ΔL=-0.76 and -1.2, respectively) after four weeks.
    • The color changes and staining produced by the new NSF formulations were significantly less than those of the commercial agents (p<0.001).

    Conclusions:

    • The novel nano-silver fluoride formulations demonstrate significantly reduced dentin staining compared to current commercial silver fluoride agents.
    • NSF presents a viable alternative to silver diamine fluoride, offering caries management benefits with improved esthetics.
    • These findings support the potential of NSF in clinical applications where esthetic concerns are paramount.
