    Background:

    • Chronic renal insufficiency (CRI) is a significant health concern in aging populations.
    • Understanding the prevalence and causes of CRI in older adults is crucial for effective management.

    Observation:

    • The study analyzed 10,350 outpatients (Group I), 2,400 episodic inpatients (Group II), and 1,840 deceased inpatients (Group III), all over 60.
    • CRI frequency was 0.98% in Group I, 6.9% in Group II, and 14.6% in Group III.
    • Serum creatinine levels above 200 µmol/l were assessed.

    Findings:

    • Pyelonephritis (76.1%) and myelonephropathy (7.5%) were identified as the leading causes of CRI.
    • A clear correlation exists between the severity of illness and CRI prevalence, with higher rates in hospitalized and deceased patients.

    Implications:

    • The findings highlight the substantial burden of CRI in older adults, particularly in more severe clinical settings.
    • Early detection and management of conditions like pyelonephritis are vital to prevent or slow the progression of CRI.
    • Therapeutic strategies for CRI in the elderly, even in advanced stages, warrant further investigation and implementation.
