Abstract

Insights

Combining venetoclax with S63845 shows promise for enhancing anti-myeloma activity. S63845 can restore venetoclax sensitivity in resistant myeloma cells, suggesting a new therapeutic strategy.

Area of Science:

  • Hematology
  • Oncology
  • Pharmacology

Background:

  • Venetoclax (BCL2-selective inhibitor) shows anti-myeloma activity, particularly in t(11;14) myeloma.
  • Resistance to venetoclax is a clinical challenge in multiple myeloma (MM).
  • MCL1 is another key BCL2 family protein implicated in MM survival.

Purpose of the Study:

  • To investigate if combining venetoclax with bortezomib or S63845 (MCL1-selective inhibitor) can enhance anti-myeloma effects.
  • To determine if these combinations can overcome venetoclax resistance in human myeloma cell lines (HMCLs).

Main Methods:

  • MTS assay was used to determine half-maximal inhibitory concentration (IC50) values.
  • Sensitivity and resistance to venetoclax, bortezomib, and S63845 were assessed in HMCLs.
  • Correlations between drug sensitivity and gene expression (BCL2, MCL1, BCL2L1) were analyzed.

Main Results:

  • Resistance to venetoclax, bortezomib, and S63845 was observed in 75%, 25%, and 50% of HMCLs, respectively.
  • Venetoclax sensitivity correlated with higher BCL2/MCL1 and BCL2/BCL2L1 mRNA ratios and lower MCL1 expression.
  • In venetoclax-resistant cells, S63845 restored sensitivity, while bortezomib did not. The combination of venetoclax and S63845 showed stronger synergy than venetoclax and bortezomib.

Conclusions:

  • Combination of venetoclax with S63845 is a promising strategy to enhance venetoclax sensitivity or overcome resistance in myeloma.
  • This combination warrants further clinical investigation for multiple myeloma therapy, irrespective of t(11;14) status.

Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Targeted Cancer Therapies

The targeted cancer therapies, also known as “molecular targeted therapies,” take advantage of the molecular and genetic differences between the cancer cells and the normal cells. It needs a thorough understanding of the cancer cells to develop drugs that can target specific molecular aspects that drive the growth, progression, and spread of cancer cells without affecting the growth and survival of other normal cells in the body.
There are several types of targeted therapies against...

Combination Therapies and Personalized Medicine02:50

Combination Therapies and Personalized Medicine

Combining two or more treatment methods increases the life span of cancer patients while reducing damage to vital organs or tissue from the overuse of a single treatment. Combination therapy also targets different cancer-inducing pathways, thus reducing the chances of developing resistance to treatment.
The combination of the drug acetazolamide and sulforaphane is a good example of combination therapy to treat cancer. The cells in the interior of a large tumor often die due to the hypoxic and...

Drugs that Destabilize Microtubules01:10

Drugs that Destabilize Microtubules

Microtubules are dynamic structures and can be regulated by microtubule targeting agents (MTAs). Microtubule destabilizing drugs are a class of MTAs that destabilize and prevent microtubules' polymerization. Both natural and synthetic chemicals can be found under this class of drugs. Vincristine and vinblastine, two vinca alkaloids, and colchicine were among the first to be discovered. These drugs can affect cells in various ways, either by inducing a change in cell morphology, preventing...

Cancer Therapies

Cancer therapies are various modes of treatment, such as surgery, radiation therapy, and chemotherapy that are administered to cancer patients.
However, cancer treatments can pose several challenges, as therapies used to kill cancer cells are generally also toxic to normal cells. Moreover, cancer cells mutate rapidly and can develop resistance to chemical agents or radiation therapy. Besides, all types of cancer cells may not respond to the same therapy. Some cancer cells respond to one...
