Background:

  • Chronic rhinosinusitis (CRS) is a prevalent inflammatory condition affecting nasal and sinus linings.
  • CRS presents with symptoms including nasal blockage, rhinorrhoea, facial pressure, and loss of smell, with or without nasal polyps.
  • Biologics, particularly monoclonal antibodies, are emerging treatments for inflammatory conditions.

Purpose of the Study:

  • To evaluate the efficacy and safety of biologic therapies for chronic rhinosinusitis.
  • To assess the impact of biologics on disease-specific and generic quality of life, symptom severity, and disease extent.

Main Methods:

  • A systematic review of randomized controlled trials (RCTs) was conducted.
  • Included RCTs compared biologics (dupilumab, mepolizumab, omalizumab) against placebo in CRS patients with at least three months follow-up.
  • Primary outcomes included health-related quality of life (HRQL), disease severity, and serious adverse events (SAEs).

Main Results:

  • Dupilumab demonstrated significant improvements in disease-specific HRQL (SNOT-22 score), symptom severity (VAS), and reduced disease extent (CT scan scores) in patients with severe CRS and nasal polyps.

  • Mepolizumab showed potential improvements in HRQL, but its effects on symptom severity, surgery rates, and disease extent remain uncertain. Evidence for omalizumab was very limited and uncertain.

Conclusions:

  • Dupilumab is an effective treatment for severe CRS with nasal polyps, improving quality of life and reducing disease severity.
  • Mepolizumab may offer benefits for HRQL, but further research is needed to confirm its efficacy and safety.
  • The role of omalizumab in CRS treatment requires further investigation due to limited evidence.
