Summary

Pulmonary artery aneurysms (PAA) can cause severe complications. A valve-sparing David-like procedure successfully treated a giant PAA with pulmonary regurgitation and coronary artery compression.

Background:

  • Pulmonary artery aneurysm (PAA) is a rare condition often presenting late with severe complications.
  • Idiopathic pulmonary arterial hypertension (IPAH) can be associated with PAA, leading to complex cardiovascular issues.

Observation:

  • A 39-year-old female with IPAH presented with a giant PAA.
  • The giant PAA caused severe pulmonary regurgitation (PR) and symptomatic left main coronary artery (LMCA) compression.
  • Previous LMCA stenting had failed.

Findings:

  • Surgical intervention included a valve-sparing David-like pulmonary trunk reconstruction and coronary artery bypass grafting.
  • The patient's condition was successfully managed with this surgical approach.

Implications:

  • This case demonstrates the feasibility of the David-like reconstruction technique for managing giant PAAs with severe PR.
  • The findings suggest a potential surgical solution for complex PAA cases previously considered high-risk.
  • This approach may offer a valve-sparing option in PAA management.
